Discharge planning is one of the best ways to support your loved one returning home. While in the facility, a complimentary assessment is first conducted to establish a plan of action. Then, Concierge Services are customized based on specific types of discharge. We train our caregivers with the physical therapists and other medical professionals’ instructions to ensure continuity of care. This allows for smoother transitions to the home and continued progress.

Our client focus of working to maintain the highest level of independence includes our transportation and activities/social engagement program. This allows our clients to keep up with the social activities that they are used to without feeling restricted by their health or inability to drive.

Because dementia is a common diagnosis among the senior population our team of managers and caregivers have dedicated and focused training specific to dementia care. Our focus involves client support, management, documentation, as well as family coaching. We furnish our Caregivers with effective techniques to improve communication and overall quality of life.

Diabetes is a serious diagnosis among the senior population. In our Diabetic Client’s plan of care, we include medication reminders, nutritional support, portion control, meal and snack preparation and meal planning.
